Silver Oak Nursing & Rehabilitation opened its doors in 2009 with a simple belief: that older adults deserve to be cared for in a place that feels like home, by people who know them by name. Named for the strong, sheltering oak, we set out to be a steady presence for families during some of life's hardest decisions.
More than fifteen years later, that mission hasn't changed. We've grown into a full-service skilled nursing and rehabilitation community, but we've kept the close, family feel that brought our first residents through the door.

Great care comes from great people. Our team includes registered nurses, licensed practical nurses, certified nursing assistants, physical and occupational therapists, social workers, dietary staff, and activities coordinators — all working together around each resident.
What sets us apart is consistency. Our caregivers tend to stay for years, so residents see familiar, trusted faces every day and families build real relationships with the people caring for their loved ones.
