Silver Oak Nursing & Rehabilitation first opened its doors in 1991 with a simple belief: that older adults deserve to be cared for in a place that feels like home, by people who know them by name. Named for the strong, sheltering oak, we set out to be a steady presence for families during some of life's hardest decisions.
For more than three decades, that mission hasn't changed. Under our current leadership since 2021, we've continued to grow into a full-service skilled nursing, rehabilitation, and wound care community — while keeping the close, family feel that has always defined Silver Oak.
